Release of Liability

I consent to receive massage therapy services.
I understand that the services provided today are not a substitute for medical care and that my therapist is not qualified to diagnose, prescribe or treat physical or mental illness and nothing said during our session should be construed as such.
I have informed my therapist of all known medical conditions and injuries and I agree to inform my therapist of any changes in my health and medical condition in the future.
I will inform my therapist if I experience any pain or discomfort during my session and will not hold my therapist or Casa Doson responsible for any pain or discomfort during or after my session.
Prenatal Clients - Acknowlogies potential risks, and has recieved clearance from a thier healthcare provider and releases Casa Dōson and therapist of liability for any harm that may occur as a result of any service.
I assume all risks and understand that my therapist will use a wide range of massage modalities, equipment, tools and products that may cause bruising, marks, redness, pain, soreness and that massage lotions,oils, sprays, back scrubs, foot scrubs, and facial products are not guaranteed to be hypoallergenic.
I understand that the use of Infrared Sauna is at my own risk. You should not use if you have a pacemaker, are pregnant and breastfeeding, have a fever, infection or injury, have highblood preassure, a heartattack or other cardiovascular issues. Furthermore you should not use if yoiu are experiencing dissiness, fainting spells, heat sensitivity, narcolepsy or siezures or have any bleeding disorders.
I understand that my therapist will use draping during my session and expose areas of my body being worked on. Glute work will require my therapist to uncover my glutes. The following areas will always remain covered: breast, groin and medial gluteal region.
I understand that any sexually illicit requests, suggested or implied, is a reason to terminate the session with ZERO refund.
I understand that I may terminate my session at any time without any refund for the remaining time.
I understand that Casa Dōson has uneven exterior grounds and uses a staircase to reach 2nd floor treatment rooms. Caution should be used.
I agree that Casa Doson is not liable for lost, stolen or damaged items. Parking is at my own risk and i must park only in designated areas to the left side of the building. Casa Doson validates parking in designated area only and is not reponsible for a lost parking ticket.
By checking the release of liability box, I voluntarily accept and assume all risks and waive and release Casa Dōson Wellness Therapy, owners, my therapist, employees, agents or representatives and facility from any and all liability, past, present and future relating to massage, bodywork and spa services.
